Passport Applications
An appointment is not required but is recommended. To schedule your appointment, call 941-861-7400.
The application process takes about 20 minutes per applicant. The Clerk and Comptroller's office begins accepting passports and taking passport photos at 8:30 a.m., Monday through Friday. Consult our Holiday Schedule and Office Closures before visiting our office locations.
Remember that two, separate forms of payment are required for each application. To simplify the payment process, we recommend that you bring your checkbook to pay all fees.
Application Fee, payable to U.S. Department of State
Check, Money Order, or Cashier's Check only. Include the applicants name and date of birth on the front of the payment. Each person must have one Application Fee payment to send along with their application.EXAMPLE: If you are a family of 3, you will need 3 separate checks, each made payable to the U.S. Department of State, for each individual Application Fee.
Execution (Acceptance) Fee, payable to Sarasota County Clerk and Comptroller
All forms of payment accepted. Credit card payments are subject to a 3.5% processing fee. Each person must pay an Execution Fee.EXAMPLE: If you are a family of 3, you will need to pay 3 Execution Fees. However, the Clerk and Comptroller allows you to pay all 3 Execution Fees with one check.
This bundling of the execution fee is allowed, no matter what form of payment you chose to use.)We are applying for our passports together. Will we receive our passports together in the same envelope?
No. each person's passport, as well as each person's documentation, will be mailed in separate envelopes.You must bring one of the following to send with your passport application packet.
- Original or certified copy of your Birth Certificate. A photo copy is not accepted.
- Original Naturalization certificate. A photo copy is not accepted.
- Previously-issued passport, if you have one.
View a complete list of primary or secondary citizenship evidence accepted by the U.S. Passport Agency when applying for a passport. You must have original or certified copies. A photocopy of the front and back of each document is also required. All original documents will be returned to you, and will arrive separate from your U.S. Passport.
All minors, and both of their parents or legal guardians must appear when applying for the minor's passport. However, the U.S. Passport Agency understands that sometimes one or both parents or legal guardians may be unavailable to appear to show parental consent.
No matter the situation, you will need to include additional documentation with the minor's passport application. Learn more about what documents are required when one or both parents or legal guardians are not available.

The U.S. Passport Agency offers two types of passport travel documents: the traditional Passport Book and a Passport Card. Both are official U.S. Passports and valid for the same amount of time. You can apply for a Passport Book, a Passport Card, or both.
U.S. Passport Book U.S. Passport Card - Can be used for domestic air travel.
- Can be used for international travel by air, sea, or land
- Can be used for domestic air travel only.
- Cannot be used for international air travel.
- Can be used for land border crossings and sea ports-of-entry, for travel to Canada, Mexico, Bermuda, and the Caribbean.
- Commonly used for cruise travel.
- Eligible for expedited processing for an additional fee; includes overnight priority return delivery.
- Eligible for expedited processing for an additional fee; not eligible for overnight return delivery.
- Wallet-sized card
- Less expensive
The oath you will be asked to answer is as follows:
"Do you swear or affirm the information in your application is true and correct, and that the picture you are submitting is a true likeness of yourself?"
We recommend that you apply for your passport well in advance of any travel plans. Busy travel seasons can affect how long it takes for your application to be processed by the U.S. Passport Agency.
Arriving early at the Clerk and Comptroller's office allows you plenty of time to complete the necessary application steps.

Life-or-Death Emergencies
Life-or-death emergencies are serious illness, injuries, or deaths in your immediate family that require you to travel outside of the United States within 72 hours (3 business days).
If you have a life-or-death emergency, call the U.S. Passport Agency's 24-hour emergency number (202-647-4000). For complete requirements, review the U.S. Passport Agency's website.
Urgent International Travel Plans
If you have international travel plans in the next two weeks, consider visiting your nearest U.S. Passport Agency location. An appointment is required.

To Correct an Error on your Passport
Check the the U.S. Passport Agency for the required forms, fees, and process to correct an error on your passport. There may be no fee to correct your passport if it was issued in the past year. Be prepared to provide a certified copy of the document that shows the correct information.
To Change the Name on your Passport
Check with the U.S. Passport Agency for the required forms, fees, and process to change the name on your passport. Be prepared to provide a certified copy of the document showing your authorized name change.

Report your physical U.S. Passport as lost or stolen immediately to protect yourself from identity theft. You can replace it now or later - it's up to you! Once you report your passport as lost or stolen, it is invalid and cannot be used for international travel.
Three Ways to Report your Lost or Stolen Passport

Phone: Call toll-free 1-877-487-2778 The U.S. Department of State Passport Agency establishes all guidelines for applying, fees, forms, time frames, and how passports are processed, and is the only agency who can process passport applications and issue U.S. Passports. All final decisions to issue a passport are made by a reviewing agent at the U.S. Passport Agency.
The Sarasota County Clerk and Comptroller serves as a Passport Acceptance Facility, providing the public with Certified Passport Acceptance Agents, at two office locations, who are specially trained to assist you with the passport application process.
When you apply for a passport at the Clerk and Comptroller's office, a Certified Passport Acceptance Agent will help you to:
- Review your application for completeness;
- Verify that you filled out the correct application form;
- Discuss the details of your application to ensure it meets the U.S. Passport Agency’s requirements;
- Verify that you have the required documentation to send with your application, including any photocopies;
- Administer an oath;
- Take your passport photo, if you do not have your own;
- Advise you on the two types of passport travel documents;Review options for having your application processed timely;
- Check that your Application Fee payment to U.S. Department of State is correct;
- Receipt your Execution (Acceptance) Fee payment;
- Explain how to track the status of your passport application;
- Go over what you can expect from the U.S. Passport Agency, including how they will contact you and how you will receive your passport and returned documentation; and
- Send your verified passport application packet (forms, fees, photo, and documentation) to the U.S. Passport Agency via overnight or certified mail.
The Clerk and Comptroller cannot process passport applications, issue U.S. Passports, nor accept passport renewal applications.
If you were age 16 or older when your passport book or card was issued, your passport is valid for 10 years.
If you were age 15 or younger when your passport book or card was issued, your passport is valid for 5 years.
The Issue Date of your passport can be found on the data page of your Passport Book or on the front of your Passport Card.
- On June 1, 2009, the U.S. Government implemented the full requirements of the land and sea phase of the Western Hemisphere Travel Initiative (WHTI). The rules require most U.S. citizens entering the United States at land or sea ports-of-entry, including travel to and from Canada, Mexico, Bermuda, and the Caribbean region, which includes 17 nations, to have a Passport Book, Passport Card, or other travel document approved by the Department of Homeland Security.
